  • Appointments required for medical appointments

  • Children and adolescents under 18 must be accompanied by a parent or guardian

  • If child is in custody of a guardian, proof of guardianship must be provided on the first visit

  • Proof of income is required to qualify for a sliding-fee scale

  • Bring a list of all medications (prescribed and over-the-counter) to your appointment

  • Records and other documentation from emergency room or other doctors’ offices should be provided on the first visit.

Fees/Insurance:

  • Fees are income based. A minimum fee of $20 required at most visits

  • Accepts Medicare, Medicaid, KCHIP (Kentucky Children’s Health Insurance Program) and private insurance.

  • If an individual is already a patient at the Mission Lexington Dental Clinic, they automatically qualify for the Mission Lexington Medical Clinic. Must have a proper referral from the Dental Clinic before receiving an appointment at the Medical Clinic.
